Patient Forms

The first visit will largely be information gathering, deciding what labs to run, and covering what the first treatment steps will be. For Naturopathic visits, expect the visit to take up to 90 minutes. For Substance Dependency Treatment members, expect 1-2 hours.

To more efficiently use our time together, we ask that all new members bring their paperwork filled out for their first visit. In an effort to reduce administrative time and preserve resources, we also ask that you enroll in the practice using the online option if possible and arrive 15 minutes early for your first visit. If it is not possible to complete the paperwork prior to your visit, we ask that you arrive at least 30 minutes early.
